Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on September 12, 2024, as a part of the provisional licensure period. The on-site inspection began at 9:05 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 98 children present and 18 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included a tour of the facility, interviews, observations, and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 23, 2024. A POC submitted after this date will not appear on the public website.

Based on review of ten staff records, the center did not ensure one staff meet program leader qualifications.

Evidence:
1. The record of staff #2, staff identified as a program leader did not contain documentation to demonstrate how staff #2 meets program leader qualifications.
2. The director confirmed that staff #2 did have documentation to demonstrate program leader qualifications but could not locate the documentation.
Plan of Correction: Qualifications were available and not in record
Standard 8VAC20-780-280-B
(Repeat Violation) Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the toddler I and toddler II classrooms, bleach water was stored over the sink accessible to children.
Plan of Correction: Bleach will be locked in closets.
