Inspector Notes

An unannounced monitoring inspection was initiated and completed on November 20, 202, in response to a self-report received by the licensing office on October 30, 2024, relating to staffing and supervision and care and protection. The inspector reviewed 1 child?s record and 2 staff records on-site on November 20, 2024. This inspection included document review, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 26, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
(Repeat Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. On October 30, 2024, from approximately 5:20 PM until 5:25 PM, child #1 (age 20 months) was found in a classroom alone by a non-staff member.
2. Child #1 was able to enter the empty classroom through a Jack and Jill bathroom.
3. Staff #3 stated that staff #1 and staff #2 confirmed that they were not aware that child #1 had left the classroom.

(Repeat Violation) Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On October 30, 2024, child #1 was found unsupervised in an empty classroom for approximately five minutes.
