Inspector Notes

A monitoring inspection was initiated on 12/12/2023 and concluded on 12/12/2023 from 11:40 AM to 1:30 PM. There were two children present with one staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the five staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-160-A
Based on record review and interview, the center failed to ensure new staff provided documentation of a TB screening that was completed within the last 30 days of the date of employment.

Evidence:

1. The record for staff 4 documents the start date as 5/29/23. The TB screening is dated 8/1/22.
2. The record for staff 5 documents the start date as 10/27/23. The TB screening is dated 8/31/22.
3. The director verified the start dates and the dates of TB documentation.
Plan of Correction: Staff 4 was seasonal part-time staff over the summer and may not be returning this summer. If staff 4 returns she will be required to obtain updated TB screening before starting. Staff 5 will be required to obtain a TB screening as soon as an appointment can be scheduled. In the future the director will ensure new staff provide a TB screening that was conducted within the previous 30 days.

Based on record review and interview, the center failed to ensure a staff member obtained an updated TB screening or test at least every two years.

Evidence:

1. The TB for staff 2 was dated 7/8/21.
2. The director verified the 7/8/21 TB results are the most current the center has.
Plan of Correction: Staff 2 will obtain a TB screening as soon as an appointment can be scheduled. In the future the director will ensure staff update TB's in the required timeframe.
