During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

The notice was revised to correct a scriveners error with the date of the inspection.
The inspection findings were updated on November 3, 2025.
An unannounced inspection was initiated on September 15, 2025 and completed on October 7, 2025 in response to a complaint received by the licensing office on September 10, 2025 for allegations relating to staffing and supervision . This inspection included observations, interviews, and record review. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 14, 2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION The required ratio for children ages birth to 16 months is one staff per four children (1:4).
During the following times on September 10, 2025, the youngest child in care being under 16 months:
9:00 to 9:15 the ratio was 1:7.
9:15 to 9:30 the ratio was 1:8.
9:30 to 10:00 the ratio was 1:9.
10:00 to 10:30 the ratio was 2:9.
3:30 to 4:00 the ratio was 1:6.
4:00 to 5:00 the ratio was 1:7.
Plan of Correction: Maintain ratio at all times by limiting reservations.
