Inspector Notes

A (monitoring inspection was started at 10:30 a.m. and concluded at 12:45 p.m. There were 12 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Children were observed playing outside, inside, having lunch, and getting ready for rest time.


Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center failed to have central registry finding within 30 days of hire.
Evidence:
1. Staff #2 (DOH 5/25/22) and staff #3 (DOH 4/11/22) did have central registry findings within 30 days of hire.
Plan of Correction: There has been a change over in human resources staff. The inspector met with new staff and reviewed requirements for all background checks. Central registry checks will be sent within 7 days; the portal may be used.
