During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced on-site inspection was initiated and completed on 6/9/2025. The on-site inspection began at 1:20 and ended at 4:00. The inspector reviewed compliance in the areas listed above. There were 22 children and 5 staff present. The inspector reviewed 5 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
Child #3 did not have documentation of updated immunizations once every six months.
Plan of Correction: Parents will be asked for the updated immunization record. All files will be reviewed for needed updated records.
Standard 8VAC20-780-350-B-2
The required ratio for children ages 16 to 24 months is one staff per five children. Two staff were observed supervising 19 children (youngest child 18 months).
Plan of Correction: The director will review with staff the required ratios (including ap time) with all staff.
Standard 8VAC20-780-40-M
The center is required to keep a current list, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.
The posted list was not dated.
Plan of Correction: Allergies, sensitivities, and dietary restrictions will be listed as required -updated and dated.
Standard 8VAC20-780-420-E-1
For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child. The record shall contain the following information: amount of time slept, food consumed and the time, a description of bowel movement, developmental milestones, and for infants who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.
There were no daily records being kept for infants.
Plan of Correction: Staff will start documenting the required information for infants.
Standard 8VAC20-780-420-E-3
Centers are required to at least annually get parent confirmation that the required information in the child's record is up to date.
Child records #1-4 did not have documentation of the parent updating any required information.
Plan of Correction: Parents will be asked to provided updated information for their child's record and it will be documented.
Standard 8VAC20-780-440-E
Centers are required to have at least 12 inches of space between occupied cots.
During rest time, children were observed on cots that were adjacent to each other.
Plan of Correction: The director will review with staff the requirement of spacing of cots during nap time.
