The center is required to keep a current list, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan.

The center had a list but it was not dated.
Plan of Correction: The allergy, etc. list will be dated and kept current.

Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

A nonprescription medication was not labeled with the child's name.
Plan of Correction: All medications will be labeled with the child's name and dosage amt. and the time to be given.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center.

An over the counter medication authorization had been expired for over 4 months.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence with all the required elements.

Completed injury report forms did not have the all the required information Out of 4 reports reviewed, two had no time documented, two had no prevention listed, and 4 did not have it listed who it was reported to.
Plan of Correction: The DOE model form will now be used and staff will trained on how to compete it.
