Inspector Notes

An unannounced monitoring inspection was conducted on 06/04/2024. Two licensing staff were on site from approximately 9:33am until 3:15pm. There were 87 children present with 16 staff supervising. There was a total of eight classrooms visited, ranging in ages from infants to preschool. Children were observed in variety of activities including dancing and singing for morning circle time, yoga, art activities, outdoor play, playing with building blocks, reading, and coloring. Inspectors interacted with staff and children in care. Staff interacted with children in a positive manner.

The inspectors reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight child records and eight staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please reach out to the licensing inspector

Standard 22.1-289.035-A
Based on record review, the center failed to ensure that the Central Registry background check and Criminal History background check were repeated every five years.
Evidence:
1. Staff #6 record (DOH 08/14/2012) did not include an updated Central Registry finding. The most recent Central Registry finding was dated 07/02/2018. Leadership acknowledged that the updated results have not been received.
2. Staff #6 record contained an updated Criminal History background check on 08/31/2023. The latest Criminal History background check result was completed on 04/21/2018.
Plan of Correction: Director re-submitted central registry finding immediately. Will follow up with central registry if not received in 30 days. Director will ensure and audit each staff member is up to date on resubmissions.
Standard 8VAC20-780-500-A
Based on observation, staff failed to wash their hands before and after helping a child use the toilet.
Evidence:
1. A staff member of classroom three was observed putting on gloves to help a child use the toilet. After the child finished toileting, staff took off the disposable gloves and put on a fresh pair of gloves to assist another child with toileting.
2. No handwashing of the staff was observed before and after helping children with toileting.
Plan of Correction: Assistant teacher was re-trained on proper diapering protocols. Director has implemented checklist to ensure steps being followed in classroom.
Standard 8VAC20-780-570-E
Based on observation, the center did not ensure prepared infant formula shall be refrigerated, dated and labeled with the child's name.
Evidence:
1. In the infant room, five bottles were observed labeled with children?s name, but no dates as required.
Plan of Correction: Staff members were retrained on proper labeling. two-step verification will be implemented upon bottle retrieval by lead and assistant.
