Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone/email contacts, documents review, interviews and a tour of the program. A monitoring inspection was initiated on 9/22 and concluded on 9/30. The provider was contacted by email and a virtual inspection was conducted. There were 4 children present, ranging in ages from 3 to 10 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence
1. Caregiver #1's most recent TB test was completed on 2/5/19.
Plan of Correction: An appointment has been scheduled to have this completed.
Standard 22VAC40-111-210-A
Based on file review, a caregiver did not obtain a minimum of 16 hours of annual training.
Evidence
1. During the inspection, caregiver #1 provided documentation of having attended 14 hours of training this past year.
Plan of Correction: Caregiver #1 will complete 16 hours of training within the next year.
