An unannounced complaint investigation was conducted on March 6, 2024 in regards to a report of forbidden actions. Interviews were conducted and records were reviewed, The preponderance of evidence does support the allegation and the complaint has been deemed valid. Based on interviews, the provider punished a child for toileting accidents.
Evidence:
1. Child #1 had a toileting accident. Caregiver #1 confirmed that they lightly tapped Child #1's bottom for a toileting accident one time. Caregiver #1 confirmed that the child did not cry and Child #1 did not sustain an injury from the light tap.
2. Witness #1 confirmed that they were present at the time of the incident. Witness #1 confirmed that Child #1 did not cry and that Child #1 did not have an injury as a result.
Evidence:

Plan of Correction: The provider has confirmed that they only lightly tapped the child's bottom once as a result of a toileting accident. The child was not injured and did not cry. The provider has not punished the child for toileting accidents since the incident and will never do it again.
