Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.

Caregiver #1 did not complete the department's health and safety update course. Documentation of the last completed health and safety update course was dated over a year.
Plan of Correction: The provider will complete the department's health and safety update course. Moving forward, the provider will ensure that the training will be completed annually.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Child #6, 9 years old, enrolled for 30 months, did not have documentation of a physical examination in the record.

Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements once every six months for children under the age of two years.

Child #1, 26 months, did not have documentation of immunizations once every six months. Documentation of Child #1's immunizations was dated when the child was nine months old. There were no current immunization records in the record.
