This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and interview.
A complaint inspection was initiated on 9/2/21 and concluded on 9/2/21. A complaint incident was received by the department regarding allegations in the areas of not communicating with parents regarding communicable disease. The director was contacted by telephone to conduct the virtual investigation.
