Inspector Notes

A renewal inspection was conducted on June 15, 2022 from 11:45 A.M.-1:15 P.M. There were two children present for a total of five points, ranging in ages from one year to three years of age, with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two child records and one caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of children's records, the provider failed to ensure that all required information was in a child's record.

Evidence:
1. Child #1 and Child #2's file did not contain a policy number for medical insurance.
2. The provider verified that the policy number for the medical insurance was not in the record.
Plan of Correction: The provider will contact the parent and obtain the medical policy number and record it in the file.
