Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on-site June 15, 2023. There were no children present. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on a review of records and interview, the vendor failed to ensure caregivers who work directly with children shall, in addition to preservice and orientation training required, annually attend at least 16 hours of training, to include the department's health and safety update course.
Evidence: The record of caregiver #1 did not contain documentation of the 2022 and 2023 annual Health and Safety update training. Caregiver #1 confirmed she the training was not completed.
