Providers must keep a written record of children in attendance each day. There were no attendance records on the date of inspection. The provider 's last inspection was May 29, 2025 and the provider didn't have any attendance records prior to November 17, 2025.

Standard 8VAC20-800-60-B
Children's records must contain all the required information. The record for child#1, who was in care the morning of the inspection and had been in care for over two months, was missing documentation of the child's address, two emergency contact persons, name, home address, and telephone number of each parent who has custody, name, address, and telephone number of each custodial parent's place of employment, information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation, name and policy number of child's medical insurance, if applicable, names of persons other than the custodial parents who are authorized to pick up the child, appropriate legal paperwork when a custodial parent does not authorize the provider to release the child to the other parent, chronic physical problems, pertinent development information, and any special accommodations needed, first and last dates of attendance, parent's signed acknowledgement of the receipt of the information required by 8VAC20-800-70, proof of the child's age and identity and the names and addresses of previously attended child day care and schools as required by 8VAC20-800-90, written authorization for emergency medical care, written authorization to administer prescription or nonprescription medication, written authorization for swimming or wading, and written authorization if the child is taken off the premises.

Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children. Several prescription and nonprescription medications were in three unlocked kitchen cabinets.
