During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/8/25. The on-site inspection began at 12:00pm and ended at 3:40pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and seven staff. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/25/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
The records for Child #1 (enrolled for three months) and Child #2 (enrolled for 10 months) show that documentation of immunizations were not received before the children attended the center.
Plan of Correction: All documentation related to immunizations as required by the state board of health will be in place before a child starts.
Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.
The record for Staff #2 (employed for over a year) did not contain documentation that 16 hours of annual training was completed in 2024.
Plan of Correction: Sixteen hours of training will be completed by all staff annually, going forward, that is appropriate to age of children.
Standard 8VAC20-780-560-G
When food is brought from home, the following shall apply: the food container shall be clearly dated and labeled in a way that identifies the owner.
Five water bottles in a four year old classroom and two water bottles in a three year old classroom were not labeled, which is required.
Plan of Correction: Going forward, all food brough from home (child's home) shall be clearly labeled and dated to identify the owner.
Standard 8VAC20-780-70
The following staff records shall be kept for each staff person: documentation that two or more references as to character and reputation as well as competency were checked before employment.
The record for Staff #1 (employed for less than a month) did not contain all the required references.
Plan of Correction: Going forward, all references will be documented prior to start date.
