During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 7/8/25 in response to a complaint received by the licensing office on 5/15/25 relating to allegations of nutrition and food services and toileting procedures. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/25/25. A POC submitted after this date will not appear on the public website.

When centers choose to provide meals or snacks, the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
On 7/3/25, the snack provided was cupcakes and ice-cream. All children were not given what was listed as snack on the menu or the nutritional components required for snack time.
On the day of the inspection (7/8/25), children were served a snack other than what was listed for the day's snack. However, the snack was not listed as a substitution.
Plan of Correction: Going forward, all snacks or meals will follow the most recent age appropriate nutritional requirements of the child and adult care food program of USDA. All snacks and all components, including substitutions.
