Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 9/30/2021 and concluded on 10/4/2021. The assistant director was contacted by telephone and a virtual inspection was conducted. There were 50 children present with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain documentation from each staff member of a negative tuberculosis (TB) screening or test within 21 days of employment.

Evidence: Staff #1 (Date of Hire: 7/7/21) had a TB test on file dated 10/2/21. Staff #2 (Date of Hire: 7/6/21) had a TB test on file dated 10/2/21. Staff #4 (Date of Hire: 8/26/21) did not have a TB test or screening on file. Staff #5 (Date of Hire: 8/3/21) had a TB test on file dated 9/30/21.

Based on record review, the center did not obtain sex offender registry search results prior to hire or request the results of an out of state central registry search by the 30th day of employment for staff who have resided outside the state of Virginia within the last 5 years.

Evidence: Staff #3 (Date of Hire: 8/31/21) indicated on a signed Sworn Disclosure Statement dated 8/23/21 that they have resided in Florida within the last 5 years. There was no documentation of a Florida central registry search request or the results of a Florida sex offender registry search. Staff #4 (Date of Hire: 8/26/21) indicated on a signed Sworn Disclosure Statement dated 8/20/21 that they have resided in Oklahoma within the last 5 years.
