Inspector Notes
An unannounced complaint inspection was initiated together with licensing inspector Morgan Bryson on 1/12/23 from 10:00am to 1:30pm in response to a complaint received in the licensing office regarding forbidden actions by a staff members, supervision and ratios, and nutrition concerns. There were 83 children ages 3 months to age 4 years old supervised by 16 staff. Interviews were conducted with staff, policies and procedures were reviewed, all classrooms were observed, and documentation was viewed. Children were observed participating in art activities, floor play and table activities. Lunch was being served during inspection and included meatballs, rice, pears, peas and milk. Based on the preponderance of evidence collected throughout the investigation, the complaints regarding forbidden actions and nutrition were determined to be invalid. The complaint regarding supervision and ratios was determined to be valid. A citation was issued as a result of the findings of the complaint investigation. Another unrelated citation was discovered during the inspection. The exit interview was conducted on 1/31/23 with a manager. 4Evidence: The Director and Toddler room staff all stated that there has been no program lead in the Toddler classroom since July of 2022 when the Program leader left.
Evidence:
In the Two-year-old classroom, the center was staffed with 3 teachers with 18 children ranging in ages from 1 year and 11 months to 2 years and 10 months. The staff to child ratio is based on the youngest child in the group which would make the appropriate ratio for this classroom 1:5.
Evidence: Child #1 (DOB 01/16/2021) was assigned to a Two-year-old classroom containing 18 children and 3 teachers and has been assigned to this classroom since 1/3/2023. The child?s file did not contain written parent permission to be re-assigned to a different age group. The child?s file did not contain a written assessment by the director and program leader.
Evidence:
1)In the Infant C classroom, there were 4 children present, but the written record reflected there were 5 children present.
2)In the PreK A classroom, there were 15 children present, but the written record reflected there was 14 child present.
