Inspector Notes

An unannounced monitoring inspection was conducted on 03/22/2023 from 10:15am to 12:15pm as a result of 2 self-reported incidents involving supervision. The physical plant, attendance, 1 staff records, 1 child record, injury reports and policies were reviewed. There was 1 violation directly related to one of the reported incidents and another violation written that was discovered in the process of the investigation.

Based on review of 1 staff record, the center did not obtain the results of a criminal history record check from all states in which staff members have resided within the last five years prior to employment.

Evidence: The record for Staff #1 (date of hire 11/7/22) did not contain documentation of a completed criminal history record check from all states where they resided within the last five years prior to employment.

Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence: In the Toddler classroom, there was a large plastic play kitchen set that was unsecured that fell on a child, causing bruising on the child?s forehead.
