Inspector Notes

A complaint investigation was initiated on 4/12/2023 at 11:45am to 1:00pm. The complaint consisted of allegations received by the Licensing Office regarding an incident of possible forbidden actions. The physical plant, attendance, staff statements, 1 staff record, and 1 child record were reviewed. Based on staff interviews and review of attendance for the classrooms, the preponderance of evidence does not support the allegation. Therefore, the complaint is determined to be not valid. A violation was cited during the course of the investigation not directly related to the complaint. Areas of non-compliance are identified in the Violation Notice.

Based on file review and staff interview, the center failed to ensure that each grouping of children had at least one staff member that met the qualifications of program leader or program director regularly present.

Evidence: The Director and Staff #1 stated that Staff #1 was alone with the School age classroom on 4/6/23 and Staff #1 does not have documentation on file that meets the qualifications of a program director or program leader.
Plan of Correction: Ensure that at least one staff member meets the qualifications of Lead teacher is regularly present.
