Inspector Notes

An unannounced monitoring inspection was conducted on 6/6/23 at 9:00am to 10:45am to investigate a self-reported incident regarding the care and protection of children in the infant room after a child was fed breast milk that belonged to another child. Interviews were conducted with 3 staff members. Attendance, incident documentation, 2 child files, and 2 staff files were reviewed. The infant classroom was observed. An exit interview was conducted with management on 6/9/23.

Based on interviews and documentation, the center did not ensure the care, protection and guidance of all children in care.

Evidence: On 5/30/23 at approximately 4:00pm, Staff #1 and Staff #2 were in the infant classroom with 7 infants when Child #1 became hungry and a bottle was placed in the bottle warmer to prepare for the feeding. Once the bottle was ready, Staff #1 fed Child #1 the bottle in the warmer that contained Child #2?s bottle of breast milk. When interviewed, Staff #1 stated they did not read the label on the bottle before feeding it to Child #1.
Plan of Correction: Ensure that all staff members read, understand, and cross-check all bottles fed to infants using the Name-to-Face procedure.
