Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/22/24 and completed on 8/22/24. The on-site inspection began at 10:30am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 88 children present and 20 staff. The inspector reviewed 8 children?s records and 7 staff records [on-site] . This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Prior to hire, the center is required to request a criminal record check from any state in which the individual has resided in the preceding five years. A criminal record check was not requested for Staff 7 prior to hire.
Plan of Correction: A request for the criminal record check was initiated during the licensing inspection. The state's policy states that a third party cannot request a criminal record check. The statement was printed and placed in the staff member's file.
