Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.

1. A bicycle and scooter on the playground were observed to have rusted.
2. Along the back playground fence is an area of ripped and exposed landscaping fabric above holes, presenting a tripping hazard.
Plan of Correction: Upon receiving the observation, a maintenance ticket was immediately submitted to facilities to address the tripping issues. The rusted bike was thrown out. The damaged landscaping fabric along the back fence will be removed and replaced to eliminate the ripping hazard. We will ensure both areas are safe and maintained in good conditions moving forward.

The staff-to-children ratio for children aged 16 to 24 months is 1:5.

Two children aged 22 months were observed in a grouping of 8 children with one staff member.
The children had been moved from their primary classroom for part of the morning and lunch.
Plan of Correction: The children were temporarily grouped together that time, which caused the staff-to-child ratio to fall out of compliance moving forward, staff will ensure that proper ratios are maintained at all times, even during transitions, meals, and temporary classroom changes. We have reviewed ratio requirements with all staff and reminded them that when combining groups, and additional staff member must be present if the children are under 24 months are included. The schedule and classroom coverage plan have been adjusted to prevent reoccurance.
