Inspector Notes

An unannounced monitoring inspection was conducted from 11:30 am - 2:00 pm with staff. There were 46 children, ranging from four months to five years of age, present with seven staff supervising. Infant care was observed and older children were observed finishing lunch, washing hands and napping. Lunch served today: taquitos, corn, mixed fruit and milk. Five child records and six staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, four medications and authorization forms, one allergy care plan and the annual fire and health inspections. Two playgrounds were also observed.

Standard 8VAC20-780-280-B
Based on observation, the facility failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: there was a bottle of sanitizing solution in an unlocked cabinet below the changing table in the infant classroom.

Standard 8VAC20-780-440-L
Based on observation, and staff interview, the facility failed to ensure that pillows are not used by children under two years of age. Evidence: in the infant and toddler class, children under age two were observed laying on cots with blankets that had pillows attached.

Based on observation, the facility failed to ensure that children?s hands were washed as required. Evidence: In the infant classroom two infants were observed eating lunch and their hands were not washed after as required and two children did not have their hands washed after having a diaper change.

Standard 8VAC20-780-500-B
Based on observation, not all disposable diapers were disposed of in a storage system that was used in such a way that neither the staff member's hand nor the soiled diaper touches the disposal system during disposal. Evidence: staff in the infant classroom were observed using their hand to open the trashcan lid to dispose of diapers.

Standard 8VAC20-780-80-A
Based on review of the written attendance record, it was determined that the facility did not maintain a written record of daily attendance, for each group of children, that documents the arrival and departure of children in care as it occurs. Evidence: In the infant classroom, three children were observed however no children were included on the attendance record for 11/8/2023. In the older infant/toddler classroom five children were observed however six children were listed as present on the attendance record. In the preschool 2 classroom, eight children were observed however only six children were included on the attendance record. In the preschool 1 classroom, six children were observed however only five children were included on the attendance record. In one prek classroom, eight children were observed however only seven children were included on the attendance record. In a second prek classroom ten children were observed however only five children were included on the attendance record.
