Inspector Notes

A complaint investigation was initiated on September 22, 2022 and concluded on November 15, 2022 in response to a complaint received by the department on September 2, 2022 regarding behavioral guidance. The inspector conducted an onsite unannounced inspection on September 22, 2022 from approximately 10:30am until 2:30pm and November 15, 2022 from approximately 1:00pm until 1:45pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of evidence gathered during the investigation does support the complaint; therefore, the complaint was determined to be valid.

Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on interview, documentation review and observation, one staff member did not interact with children to provide needed help, comfort, and support.

Evidence:
1. [Witness A] stated Staff #1 spoke harshly or yelled at children in the Infant Room (eight months to 16 months of age) on August 31, 2022. Witness A stated Staff #1 made the statements to the children in a stern voice: ?You really just went pee in your pull up after we put you on the potty?; ?Ma?am, your key just fell off and I?m not too happy?; ?I knew I had people with nasty noses?, ?Don?t tell me know?.
2. Staff #2 confirmed Staff #1 often used strong language that might take as negative. Staff #2 stated Staff #1 would yell to the Infants including statement like ?You?re not the teacher? and ?You stop it?.
3. A Plan of Action written by Staff #1 stated ?I am also going to work on not being negative and being more sensitive to the children?
