Inspector Notes

An unannounced monitoring inspection was conducted on August 29, 2023 from approximately 10:00 am to 1:00 pm. There were 50 children in attendance and a total of 13 staff present. 5 staff records and 5 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and playgrounds. The children were observed eating lunch and taking a nap, while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and sign and return it to me within 5 business days from today, 09/11/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on review of medications, it was determined that prescription medication was not maintained in the original, labeled container.
Evidence: The long term prescription medication for child#5 was not in the original container with a prescription label attached.

Based on record review, and interview, the center failed to ensure that at least one staff in each classroom or area where children are present shall have current certification in CPR.
Evidence:
The record for staff #1, date of employment 06/13/2023, did not contain documentation of current certification in CPR. Interview with administrators confirmed that staff #1 did not have current certification in CPR. Staff #1 was alone in classroom #6 with 5 preschool age children.

Based on observation, and interview, the center failed to ensure that when food is brought from home it shall be clearly dated and labeled.
Evidence: The refrigerator in classroom #2 contained several bottles with infant formula that did were not dated. Staff confirmed that the bottles were not dated.
