Inspector Notes

An unannounced monitoring inspection was initiated on October 3, 2024 and completed on October 17, 2024 in response to a self-report received by the licensing office on relating to forbidden action. The inspector reviewed one child?s record and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (October 30, 2024). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures. The center?s policy for behavior guidance states ?The following actions and threats are forbidden: physical punishment, striking a child, roughly handlings, or shaking a child, restricting movement through binding, or tying forcing a child to assume an uncomfortable position or exercise as punishment. Documentation indicated Staff #1 was terminated due to not following the behavior guidance policy on September 26, 2024.
Plan of Correction: At orientation, each staff is trained in center policy for behavior guidance and the standard on forbidden actions which is updated annually, On 10/03/24 all 25 center staff were re-trained in behavior management procedures and the standard on forbidden actions and consequences of such.
Standard 8VAC20-780-410-1
The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment. On September 26, 2024 at approximately 4:40pm, Staff #1 was seen striking Child A (13 months of age) on the side of the head two to three times.
Plan of Correction: Staff #1 employment terminated immediately. All remaining staff were re-trained in behavior management procedures and the standard on forbidden actions, consequences of such.
