During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 10/6/2025 and completed on 10/21/2025, with local agencies, in response to a self-report received by the licensing office on 9/30/2025 relating to allegations of forbidden actions. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/31/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection and guidance.

Staff #2 grabbed Child #1 (age 3) forcefully by the arm on at least one occasion to move the child from one area of the classroom to another. Child #1 reported that staff #2 hurt them on the arm.
Plan of Correction: The center fulfilled all requirements, however, staff #2 ignored their training on this day resulting in termination of employment. For transparency purposes, all parents were notified in writing of the incident. After fulfilling all requirements for employment as per state regulations, the center was cited for the incident. The child remains enrolled.
Standard 8VAC20-780-40-E
The center shall be in compliance with the center?s own policies and procedures that are required by the standards.

The center?s required discipline policy states that roughly handling a child is forbidden.

Staff #2 grabbed Child #1 (age 3) forcefully by the arm on at least one occasion to move the child from one area of the classroom to another.
Plan of Correction: The center does comply as all staff are trained at orientation (per state requirements) and 2 times annually on forbidden actions, behavior guidance, and mandated reporting. Unfortunately, staff #2 chose to disregard center and state policies and regulations resulting in termination of employment. For transparency purposes, parents were notified in writing of the incident. The child remains enrolled.
Standard 8VAC20-780-410-1
The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

Staff #2 grabbed Child #1 (age 3) forcefully by the arm on at least one occasion to move the child from one area of the classroom to another.
Plan of Correction: Employee #2 employment was terminated immediately. Administrator self reported incident to VDOE specialist. A training was held that same evening on forbidden actions, behavior guidance, and mandated court reporting. For transparency purposes, all parents were notified in writing of the incident. The child remains enrolled.
