Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated on October 4, 2021 and concluded on October 12, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 63 children present, ranging in ages from 4 months to age 4, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence: Emergency bottles and formula were observed for infants the one infant did not have the required emergency formula.
Evidence: The licensee did not obtain the required out of state background check for staff #5 for Florida where they had lived within the last 5 years. Staff #5 date of hire was 4-27-2021.
1) There were no results of a sex offender check available for review. The sex offender check is required prior to the first day of employment.
2) There were no results of a CPS check available for review. Staff #1 documented the check had been submitted but not followed up within the required time frame.
