Inspector Notes
A subsidy health and safety (SHSI) inspection was initiated on March 29, 2023 and concluded on March 30, 2023. There were 83 children present with 15 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard 8VAC20-780-600-D
Based on a record review and staff interviews, the vendor did not ensure staff completed who work directly with children shall have obtained within 90 days of the date of hire a current certification in first aid appropriate to the ages of children in care.
Evidence: The record for Staff #3 (date of hire 11-28-22) did not include documentation of First Aid certification.

Standard 8VAC20-790-600-D-1
Based on a record review and staff interviews, the vendor did not ensure staff completed who work directly with children shall have obtained within 90 days of the date of hire a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in- person competency demonstration.
Evidence: The record for Staff #3 (date of hire11-28-2022) did not include documentation of CPR certification.

Standard 8VAC20-790-600-F
Based on record review and staff interview the vendor did not ensure Staff who work directly with children shall, in addition to preservice and orientation training required, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence: Director confirmed that Staff #2 did not have documentation for their 2022 health and safety update course.
