Inspector Notes

An unannounced complaint inspection was initiated in the center on October 2, 2023, continued on October 5, 2023 and completed on October 11, 2023 regarding concerns of Staffing/ Supervision, Staff Qualifications/Training and Programs.
During the inspection the following were completed: staff interviews, review of records / documentation, review of video footage. There were also observations of parent pick up, child to staff ratio and staff and child interactions.

The preponderance of the evidence gathered during the investigation does support the allegation related to Staffing /Supervisions and therefore, the complaint is determined to be valid.
The inspection was amended on 10-17-2023 related to a technical error.

Basse on observation, staff interviews and documentation review, the licensee did not ensure to follow the required ratio of 1 staff for every 4 infants in care.

Evidence: On 10-2-2023 at approximately 5:05 PM, in the fellowship hall 9 children age range 14 months (Child #1) to age 2 were observed with only 2 staff (Staff #1 and Staff #2), this exceeds the required ratio of 1:4.

Based on observation, staff interviews and documentation review, the licensee did not ensure to follow the required ratio of 1 staff for every 10 children ages 3 years old up to school age eligible.

Evidence: On 10-2-2023 at approximately 5:05 PM, in the fellowship hall 12 children ages 3 and 4 were observed with only 1 staff (Staff #3), this exceeds the required ratio of 1:10.
