Inspector Notes
An unannounced monitoring renewal inspection was started in the center on October 5, 2023 from 12:45 PM until 4:00 PM and concluded on October 11, 2023. At the time of entrance, 71 children were in care with 14 staff members present. The sample size consisted of 7 children?s records, 7 staff records and the background checks of 9 other persons. Children were observed during rest period preparing for afternoon routine and the infants were observed freely playing and eating.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) was request by the
30th day of employment for any state where they lived within the last five years from date of
hire. (DOH)
Evidence:
1.Staff #3's record did not include an out of state CRC for Alaska (DOH 9-1-2023).
2.Staff #15?s record did not include an out of state CRC for Pennsylvania (DOH 9-18-23) .
3.Staff #6 record did not include and out of State CRC and CPS for South Carolina (DOH
7-3-2023).
all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept
confidential in each room or area where children are present.
Evidence: Child #4 has an allergy to a medication (amoxicillin), and it was not listed on the allergy list.
counter skin products shall be used in accordance with the manufacturer's recommendations. Nonprescription drugs and over-the-counter skin products shall not be
kept or used beyond the expiration date of the product.
Evidence: In the toddler room, 2 out of 5 of diaper creams observed had expired, (Desitin
expired 6-2023 and Boudreaux Butt Paste expired 4-2023).
Based on record review and staff interviews, the licensee did not ensure staff records contained all required documentation.
Evidence:
1.There were no references available for review for Staff #1, Staff #4, Staff #5, and Staff
#6.
2.The references for Staff #2 and Staff #7 did not include the signature of the person
conducting the reference check.
3.There was not documentation to demonstrate Staff #1 and Staff #2, designated program
leads have the experience required for the position
