Inspector Notes

An unannounced inspection was initiated on 6/23/2025 and completed on 8/26/2025 in response to a complaint received by the licensing office on 5/6/2025 relating to allegations of Administration, Physical Plant, Staffing and Supervision, Programs, and Background Checks
The inspector reviewed 1 staff records electronically on 7/16/2025. Children's records were not reviewed as part of this complaint inspection. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

The center is required to request an Out of State Child Abuse and Neglect Search prior to employment for any staff who has resided outside of Virginia within the last five years.
Staff # 1, who resided outside of Virginia within 5 years of hire, had no documentation available to demonstrate that an out-of-state search of the abuse and neglect registry was requested. Staff #1 was employed and caring for children for over 4 months without results of an out-of-state child abuse and neglect search.
Plan of Correction: Staff #1 had already left employment and was no longer working at the center at the time of the inspection. All background checks, for Virginia or Out-of-State, will be requested prior to the first date of employment and documentation of such requests will be kept in the staff record.
