The center is required to obtain a completed sworn statement prior to the employee's first day of employment. There was no documentation of sworn statement completed by Staff #3, who is currently working.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, employed for more than 3 months, did not have documentation of requesting a central registry check or sex offender check from a state that she lived in the past 5 years.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Staff #2 and staff #6 have been employed for more than 90 days and have not completed Virginia Department of Education-sponsored orientation course.
