Inspector Notes

A monitoring inspection was conducted on 11/1/22 from 10:30 am to 12:00 pm. There was 1 child present with 2 staff supervising. The child was observed in play and learning activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming and special care and emergencies. 1 child's record, 3 staff records, and the center's emergency drill records were reviewed. There were no medications on site.

Standard 22.1-289.035-A
Based on records review, the center did not obtain a repeat Sworn Disclosure Statement (SDS) for each employee, every five years.
Evidence: Staff # 1's most recent SDS was dated 10/3/17, and was not repeated every 5 years.
Plan of Correction: The SDS was updated during the inspection.
Standard 22.1-289.035-B-4
Based on records review and staff interview, the center did not obtain the results of an out of state criminal record check for an employee, prior to the first day of employment.
Evidence:
1. Staff # 2's first day of employment was 10/17/22. Staff # 2's record indicated that she had resided in Wisconsin within the last 5 years.
2. Staff # 3 stated that the CRC for Staff # 2 was not submitted, and the results were not obtained before Staff # 2's first day of employment.
Plan of Correction: The out of state CRC will be submitted within ten days of the inspection. The inspector will be notified once e the results have been obtained.
Standard 8VAC20-770-60-C-2
Based on records review and staff interview, the center did not obtain results of a Virginia central registry search (CRS) for an employee within 30 days of employment.
Evidence:
1. Staff # 4's first day of employment was 7/25/22.
2. A copy of the first CRS mailed for Staff # 4 showed that it was mailed on 8/2/22.
3. Staff # 3 stated that due to some error the form was returned.
4. Staff # 3 hired Staff # 2 in October and mailed the forms for Staff # 2 and Staff # 4 together on 10/26/22.
5. Staff # 4's CRS results were not obtained within 30 days of the first day of employment.
Plan of Correction: The form has been submitted. The center will do a follow up with them and notify the inspector when the results have been obtained.
Standard 8VAC20-780-130-A
Based on records review, the center did not obtain documentation of immunizations for each child before the first day of attendance.
Evidence: Child # 1's first day of attendance was 10/25/22. Documentation of immunizations for Child # 1 were not on obtained to this date.
Plan of Correction: The parents will be asked to provide us the immunization records.
Standard 8VAC20-780-160-A
Based on records review, documentation of negative tuberculosis (TB) screenings for staff were not obtained within 30 days prior to the first day of employment.
Evidence:
1. Staff # 2's first day of employment was 10/25/22. Staff # 2's TB screening was not obtained to this date.
2. Staff # 4's first day of employment was 7/25/22. Staff # 4's TB screening was dated 8/1/22.
Plan of Correction: TB screenings will be obtained within the 30 days and the missing TB screening will be obtained.
Standard 8VAC20-780-70
Based on records review, the center did not obtain two reference checks for staff before employment.
Evidence: Staff # 2 was hired on 10/25/22. There was only one reference check in Staff # 2's record.
Plan of Correction: One more reference check will be obtained and kept on file.
