Inspector Notes

A monitoring inspection was conducted on 10/30/23 from 10:45 am to 12:00 pm. There were 12 children present, with 4 staff supervising. The children were observed engaged in movement activities, arts and learning. The infant classroom was open today. There were no children with allergies or medications. A total of 2 children's records, 3 staff records, attendance and emergency drill records were reviewed.

Based on record review, the center did not obtain documentation of a physical exam for each child within 30 days of enrollment.
Evidence:
1. Child # 2's first day of enrollment was 7/10/23.
2. Child # 2's record did not have documentation of a physical exam.
Plan of Correction: The child does not attend everyday. We will get the physical exam copy from the parents.

Based on records reviewed, the center did not have complete information for each child's record.
Evidence: Child # 1's record was missing one emergency contact's address and complete information for the second emergency contact.
Plan of Correction: The information will be collected from the parents.
