Inspector Notes

An unannounced renewal inspection was conducted with the provider beginning at 12:30 PM and ending at 2:00 PM. There were 5 children, ages 8 months, 16 months, 2, 2, and 2 years old, in the care of 2 caregivers for a total of 13 points. The children were observed playing with age appropriate toys. Four children?s records were reviewed. Two caregiver?s records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Three medications were reviewed and in compliance. The most recent emergency evacuation drill was documented as 5/13/22. The most recent shelter-in-place drill was documented as 4/22/22. The most recent lockdown drill was documented as 1/10/22.

Standard 8VAC20-800-210-A
Based on a review of documentation and provider interview, the provider and assistant did not have documentation of a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities. Evidence: The provider and assistant did not have documentation of 16 hours of training in their records.
Plan of Correction: The provider stated that she and the assistant had completed training hours online but did not print out the certificates. The provider stated that she would print out the documentation and place it in their records.
Standard 8VAC20-800-530-F
Based on observation, the provider did not ensure that no soft bedding of kind is used under or around infants including but not limited to, pillows or stuffed toys. Evidence: Child # 1 was observed in his crib with a stuffed animal.
Plan of Correction: The provider stated that she would not allow the child to nap with a stuffed animal moving forward.
Standard 8VAC20-800-690-D
Based on observation, the provider did not ensure that the diapering surface was nonabsorbent and washable. Evidence: The changing pad had several small tears making it absorbent.
Plan of Correction: The provider stated that she would purchase a new changing pad.
Standard 8VAC20-800-90-A
Based on a review of documentation, the provider did not obtain documentation of immunization for all children prior to their first day of attendance. Evidence: Child #1 and child #2 did not have immunization prior to attending the family day home.
Plan of Correction: The provider stated that she had requested the documentation from the parent. The provider will request the missing documentation from the parent.
Standard 8VAC20-800-960-E
Based on observation, not all of the refrigerated bottles were labeled with the child's full name and the date. Evidence: The was a bottle in the refrigerator that was not dated.
Plan of Correction: The provider dated that bottle during the inspection and stated that she would check them for labels and dates.
