Inspector Notes

An unannounced renewal inspection was conducted on June 2, 2023, from 11:23am to 1:18pm with the provider. There were five children in care, ages 11 months, 2, 2, 3 and 3 years old, were in the care of one caregiver for a total of 12 points. The children were observed playing with age-appropriate toys and singing songs with the assistants. Five children?s records were reviewed. The provider record and two staff records were reviewed and in compliance. The provider has current certification in CPR and First Aid. The provider does not give medication at this time and is aware that Medication Administration Training (MAT) is required to do so. The first aid kit, flashlight, and battery-operated radio were observed. The most recent emergency evacuation drill was documented as 5/5/2023. The most recent shelter-in-place drill was documented as 1/20/2023.

Based on a review of documentation, the provide did not obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence: Child A and child C's records were missing documentation of a physical examination.
Plan of Correction: The provider will request the documentation from the parents and place it in the children's record when it is received.
Standard 8VAC20-800-70-B
Based on review of documentation, the provider did not obtain the parent's written acknowledgement of the receipt of the information. Evidence: Child A, child B, child C, child D, and child E's records were missing signed written acknowledgements.
Plan of Correction: The provider will have the parents sign the acknowledgement form and place it in each child's record.
