Inspector Notes

An unannounced monitoring inspection was conducted on January 24, 2024 from approximately 10:00 am to 11:35 am. Fourteen staff were present supervising sixty-two children ranging in age from infants through preschool. Additional staff were on site for administrative and kitchen duties. Children were observed playing freely inside and outside as well as participating in circle time. Six child and six staff records were reviewed. Violations were found and are noted on the violation notice.

Based on review, facility did not ensure 1 of 6 staff records have a required updated background check dated within the past 5 years.

Evidence: The record for staff #4 has a most recent OBI national fingerprinting letter dated 11/26/2018 which is more than 5 years old.
Plan of Correction: A OBI fingerprinting request will be completed and submitted immediately.
Standard 8VAC20-780-160-A
Based on review, facility did not ensure 2 of 6 staff have a negative tuberculosis (TB) screening or testing prior to beginning employment and dated within 30 days.

Evidence: The following was found during staff record review:
1. The record for staff #5 (start date 1/22/2024) did not have a negative TB screening or testing.
2. The record for staff #6 (start date 1/4/2024) has a TB screening dated 1/8/2024.
Plan of Correction: TB test/screening will be completed. In the future, TB will be done prior to beginning employment.

Based on review, facility did not ensure 1 of 6 staff records has an updated tuberculosis (TB) screening or testing every 2 years.

Evidence: The record for staff #3 has a most recent TB testing dated 11/22/2021 which is more than 2 years old.
