Inspector Notes

An unannounced, on-site monitoring inspection was initiated on September 24, 2024 and completed on September 25, 2024. The on-site inspection began at 12:40 pm and ended at 2:45 pm. The inspector reviewed compliance in the areas listed above. There were 62 children present and 16 staff. The inspector reviewed 6 children?s records and 6 staff records on-site and electronically on September 25, 2024. This inspection document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 2, 2024. A POC submitted after this date will not appear on the public website.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #4, #5 and #6, who had been employed for three to four weeks, did not have a central registry search. The requests were submitted three to ten days after beginning employment.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for child #1, who had been in care for a month, did not have an immunization record.

Center shall have a physical examination for each child before the child's attendance or within 30 days after the first day of attendance. The record for child #1, who had been in care for 6 weeks, did not have a physical.

Standard 8VAC20-780-160-A
(Repeat violation) Center shall obtain documentation of a negative tuberculosis for each staff at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the past 30 calendar days. The records for staff #1, #3, #4, and #5 had tuberculosis screening completed four days to three months after beginning employment. The record for staff #6 had a tuberculosis screening completed a year prior to beginning employment.

Hazardous substances such as cleaning materials shall be kept in a locked place. Disinfectant wipes and cleaning solutions were observed in classrooms on shelves and on filing cabinets. The items were labeled "warning/caution" and "keep out of reach of children".

Center shall ensure there is a nonabsorbent surfacing for diapering children. In the Infant 2 classroom, the diapering pad was covered with a cloth cover causing the surface to be absorbent.
