During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/22/2025 and completed on 10/2/2025. The inspector reviewed compliance in the areas listed above. There were 56 children present and 14 staff. The inspector reviewed 5 children?s records and 5 staff records on-site This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

(Repeat Violation) Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass a sufficient area of resilient surfacing to include the child's trajectory in the event of a fall. On the toddler playground, a small plastic slide and a blue two-seat rocker were observed directly on the ground without any resilient surfacing.
Plan of Correction: The toddler slide and blue rocker were removed from the toddler playground during the inspection. Staff will be reminded that these items may only be used if and when they are placed over resilient surfacing. The director is currently exploring options to upgrade the toddler playground to ensure all requirements are met for the younger children to safely play outdoors.
