Inspector Notes

An unannounced Monitoring Inspection took place on March 13, 2024 between the hours of approximately 9:15 a.m. and 10:15 a.m.There were 4 classrooms observed with a total of 33 children with 7 staff within the supervision guidelines. The children were observed participating in circle time, engaging in free play, eating and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, and fire drill log was conducted during this inspection. 2 staff records and 5 children's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain documentation of a negative tuberculosis screening at the time of employment for 1 out of 2 staff.

Evidence: Staff #2 (start date: 02/28/2022) did not have documentation of a negative tuberculosis screening on file.
Plan of Correction: TB test had fallen between files and was found already. It is in staff's file.
