During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 2/27/25. The on-site inspection began at 10:40am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 8 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-P
When an authorization for medication expires, the center shall notify the parent that the medication must be picked up within 14 days of the authorization expiring or the parent must renew the authorization.

Medication for Child 2 was onsite, but the medication authorization had expired over eight months ago. The medication was administered six times during the expired period.
Plan of Correction: The center responded with the following: The parent was contacted and a new medication administration authorization form was requested. Once obtained, the document will be placed in the child's record.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for child 1, enrolled for over 1 1/2 years, contains only one name, address and phone number of a person to call in an emergency if a parent cannot be reached, where two are required.
Plan of Correction: The center responded with the following: The parent was contacted during the inspection and a second emergency contact was obtained and documented in the child's record.
