During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/18/25. The on-site inspection began at 11:25am and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 8 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff 1's TB screening was dated eleven months prior to their date of employment.
Plan of Correction: The center responded with the following: Staff 1 will obtain a new TB screening as soon as possible and the results will be placed in thier record.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for child 1, enrolled for over 3 months, did not contain documentation of viewing proof of the child's identity and age.
Plan of Correction: The center responded with the following: A copy of child 1's birth certificate was requested at time of enrollment but was never received. An email was sent to the parent of child 1 and a copy of the child's birth certificate was again requested. Once received, it will be placed in the child's record.
