During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/10/26. The on-site inspection began at 10:45am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 7 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check every five years. A repeat central registry check for staff 1 was overdue by three months and the updated sworn statement for staff 1 was overdue by over one month.
Plan of Correction: The center responded with the following: Staff 1 has completed an updated sworn statement and a central registry search request has been submitted. Once received, the results will be placed in staff 1's record and forwarded to the licensing inspector.

The required ratio for children ages 3 years old to school age eligible is 1 staff per 10 children. At the time of the inspection, staff 1 was observed alone in one of the preschool classrooms supervising eleven children, the youngest of which was 3 years old.
Plan of Correction: The center responded with the following: The center was short-staffed on the day of the inspection. One of the children was moved to the second preschool classroom in order for both rooms to remain in ratio.
