Inspector Notes

An unannounced renewal inspection was conducted today from 9:55am-12:00pm. There were 53 children directly supervised by 12 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 6 children?s records, 6 medications, emergency drills, emergency supplies, and policies were inspected. Children were observed engaging in center play, reading books, coloring, playing with blocks, listening to story time, dancing, and playing outside. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on observation of the playground, areas and equipment outside the center have not been maintained in a safe condition.

Evidence:
1. On the School Age playground, at the bottom of the wooden bridge structure, there were 4 nails protruding that could entangle clothing or snag skin.
2. On the Infant/Toddler playground, there were four toddler bicycles with multiple cracks and holes in the plastic wheels.
Plan of Correction: Protruding nails were fixed the afternoon of the visit.

Toddler riding toys were patched the afternoon of the visit.

On the playground safety check list, items needing immediate attention will be communicated to the director daily.

Based on documentation review and staff interview, the center has not ensured that lockdown procedures are practiced at least annually.

Evidence: The last lockdown drill was conducted on 1/12/2021.
Plan of Correction: Lockdown drill was completed on Friday June 3, 2022 at 2:43. Drills will be conducted no longer than 365 days apart going forward.
