Inspector Notes

An unannounced inspection was conducted on 8/29/23 to investigate a self report received by the department related to a forbidden action. Based on evidence gathered through interviews and documentation review, the allegation could not be substantiated. There was, however, one violation cited as a result of this inspection. An exit interview was conducted on 9/29/2023.

Based on review of three staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #1 had a TB test/screening dated: 8/5/2021 and the record for staff #2 had a TB test/screening dated: 2/10/2021.
