Inspector Notes

An unannounced monitoring inspection was conducted from 11:30am - 2:00pm with the center director. There were 36 children, ranging from three months to five years of age, present with nine staff supervising. Infant care was observed, and older children were observed reading books, eating lunch, and preparing for nap. Lunch served today: vegetable beef soup, grilled cheese sandwiches, mandarin oranges and milk. Four child records and four staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, required posted information, annual fire and health inspections and two emergency medications. Based on observation, the facility failed to ensure that children's hands were washed as required. Evidence: In the infant classroom, an infant was observed eating lunch and their hands were not washed afterwards. In the toddler classroom, a child was observed having a diaper change and their hands were not washed afterwards.

Standard 8VAC20-780-500-B
Based on observation, the facility failed to ensure that all diapering procedures were followed, as required. A nonabsorbent surface (changing table) shall be used for diapering children under three years of age. Evidence: In the two's classroom, pullups were observed in a trash can in the bathroom; when asked, staff stated that they don?t use the changing table and change the children standing up in the bathroom. All six children in the two's class are younger than three years of age.

Standard 8VAC20-780-70
Based on review of four staff records, not all records contained all required information. Evidence: The records for staff #1 and staff #2, who were reported to be the lead teachers in their classrooms, did not contain verification of age requirement and documentation to demonstrate that the individual possesses the education, certification and experience required for the job position. The record for staff #3 (date of hire: 3/22/22) did not contain documentation of orientation training.
