During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/26/2024. The on-site inspection began at 10:12 a.m. and ended at 1:40 p.m. The inspector reviewed compliance in the areas listed above. There were 32 children present and 10 staff. The inspector reviewed five children?s records and five staff records on site.

This inspection included document review (i.e. Injury logs, emergency drill logs, medication authorization forms, fire inspection, and health inspection), a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/08/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-245-A
Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care.

Staff #2 obtained 13 of the 16 required hours in 2023.
Plan of Correction: We will create a better tracking system for administrators to monitor and ensure that all required annual trainings are completed, and they are appropriate to the age of the children in the teacher's care.
Standard 8VAC20-780-260-B
Annually, the center shall provide approval from the health department regarding water supply, sewage disposal system, and food service if applicable.

The center did not obtain a Report of Environmental Sanitation annually as required.
Plan of Correction: Our Virginia Health Department Inspection is attached. A report of environmental sanitation will also be obtained.
Standard 8VAC20-780-40-G
The center shall verify proof of a child?s age and identity within seven days of initial attendance.

There was no documentation of the age and identity of Child #2 or Child #4 in the records. Child #2 had been enrolled for almost four months and Child #4 had been enrolled for over three months.
Plan of Correction: We will conduct an audit of all children's files to ensure that all required information is provided within the file.
